I have built a new template from scratch. 

All submits such as comments and update quote are working but when accepting the quote it returns an Oops screen.
All details are filled that I can see. The only thing I did not include was payment facility as this wont be handled by QV.

Any help would be much appreciated.

Goodness, could be anything. A rogue missing value, something up with the HTML. If you share your HTML, I'll gladly take a quick look.

Happy to share this when it is done as believe it is something needed.

FYI the template does not like you giving css html{ height:100% } very weird.
